nesquena-hermesandwlknight 8f5ffea4a1 fix(#4729): guaranteed-exit reasoning flush in inner finally (Codex re-gate 2) ...

Codex re-gate 2: post-run flush is bypassed on exception (jumps to outer except) + retry
paths, so a buffered reasoning tail was lost before apperror there. Added a guarded
_flush_reasoning_buffer() at the top of the inner finally (guaranteed exit for
normal/exception/retry) so the tail always reaches the live Thinking view before teardown.
+ regression asserting the finally flush.
