luandnh c1c039ff80 fix: protect pipes inside backtick code spans in renderMd tables ...

The _protectPipes function protected | inside brackets but not inside
backtick inline-code spans, so || in `code || here` inside a table
cell was split into extra columns. Added a second replace for matched
backtick pairs in the same iterative loop.
